Thank you for submitting your bank’s card figures for this report.

As promised, the report which lists your bank, is now available as a PDF which can be downloaded. Adobe Acrobat Reader is required to view the document.

The PDF document you are about to download can only be used on a total of two computer systems. It can only be used on a computer system that uses a Static IP address (this means that your computer uses the same IP address each time it powers up).
Only open the PDF on the computer system you want to use to view the report. After you have saved the PDF document to the first computer system it can be moved, copied or emailed to ONE additional computer system.

2) Double-click the PDF icon to open the document on your desktop. Only open the PDF on the computer system you want to use to view the report.

3) Insert your username and password.


4) If prompted with the security warning below, please check the box "Remember my action for this site" and click "Allow."



5) Select Yes if you would like to automatically sign in to the document next time.



6) The document has successfully been unlocked. Scroll down to view the rest of the document.

7) Note: You will be prompted again for the username and password that was sent to you via email when the PDF is opened on a second computer system.
